3,5 x 4,5 cm colour photo takend against a white background and printed on quality paper
Taken en face.clearly showing the eyes and face from both sides from top of the head to the top of the shoulders with the face covering 70 to 80 percent of the photo.
The photo must be taken without a headwear or glasss
-A copy of a passport personal data page and photo
-Copy of the ID document to confirm residence within the consular district of your application
-Flight booking
Must include dated and flight numbers specifying entry end exit from Schengen states round trip.
-Hotel booking
Proof of accommodation for the entire duration of the intended stay
A confirmed hotel reservation is requested
A hotel booking must include a full name of the applicant,hotel address and phone numbers
-Travel Insurance
Traveling insurance valid for all EU or Schengen states
Minimum cover of 30 000 Euro
The policy letter must state the applicant’s name, passport number, policy number minimum of 30 000 Euro and cover for the duration of stay
-Letter confirming employment
Signed letter from employer confirming employment and indicating monthly salary, continued employment after a period of leave and duration of absence.
In case of the self employed business certificate or a letter from your accountant
-Three months bank statement
A latest bank statement must reflect a applicant’s full name and address with bank’s signature.
Business visa
Officially signed invitation letter from a company in Poland.
The invitation should include clear identification of both the entity issuing it and the invited applicant, nature, duration and venue of the event.
Applicant’s role in the event and their link between the host and the invited applicant.
If expenses are covered by by the host,this needs to be clearly established in the invitation.
If applicable proof of registration and payment is also required
Foreign entrepreneurs and representatives of foreign business entities applying for business visas have to be notified by their diplomatic mission accredited by the Republic of Poland and must have to obtain the consent of the Polish minister of foreign affairs for such visit
